CLEVELAND CLINIC PATIENT PERSPECTIVES SWEEPSTAKES RULES & REGULATIONS:
1. SWEEPSTAKES PERIOD: The Cleveland Clinic Patient Perspectives Sweepstakes (“The Sweepstakes”) periods are as follows. The Sweepstakes is being sponsored by CLEVELAND CLINIC (the “Sponsor”) and administered by Fuel Cycle, Inc. ("Fuel Cycle"). Fuel Cycle is and shall remain solely responsible for delivering the Prize(s) to the winner(s).
There are three types of sweepstakes periods:
a) Activity Sweepstakes Period – For Activity Sweepstakes, the sweepstakes period for each activity commences at 12:00:01 a.m. on the date when the first email communication is sent to Patient Perspectives Community members that notifies them of a new sweepstakes and the sweepstakes period ends at 11:59:59 p.m. on the date specified in the same email (the first email communication about the new sweepstakes provided to Patient Perspectives Community members) (the “Sweepstakes Period). Close Date Extension: The close date can be extended at the discretion of the Sponsor or Fuel Cycle with an email to community members who have not completed the activity as required. There will be no notification of the close date extension to community members who have already been entered into the sweepstakes.
c) Recruiting Sweepstakes Period – For recruiting sweepstakes, each sweepstakes period will close a month after the recruitment begins (e.g., September 1st 2022- October 1st 2022). Each period commencing at 12:01 a.m. Eastern Time and ending at 11:59:59 p.m. Eastern Time (the “Sweepstakes Period”).
2. ELIGIBILITY:
a) Activity Sweepstakes - Sweepstakes are open to all registered members, of the Cleveland Clinic Patient Perspectives Community (“Entrants”). All Entrants must be individuals legally residing in the United States and who are the age of majority in their State of residence at the time of entry into the Sweepstakes.
b) Recruiting Sweepstakes – Sweepstakes is open to all legal U.S. residents, including the District of Columbia, who are above the age of majority in the state in which they reside at the time of entry. Void in Puerto Rico, and where prohibited. Sweepstakes is subject to all local, state and federal laws and regulations. By entering this Sweepstakes, all entrants agree to comply with and be bound by these Official Rules, and to comply with all federal, state and local laws and regulations.
3. PRIZES: All Prizes to be awarded in US dollars. All Prizes are to be awarded in US dollars by Fuel Cycle’s e-gift card vendor in accordance with the Prize Awarding requirements set out below.
There are 2 (two) different types of Prizes that Entrants can be eligible for:
a) Activity Sweepstakes - TOTAL DRAWS, NUMBER PRIZES FOR EACH to be communicated to Entrants in email communications when inviting them to participate in each activity.
b) Recruiting Sweepstakes – There will be 10 (ten) draws (each a “Draw”) in the Sweepstakes consisting of $100 (each a “Prize”) for an aggregate of $1000 in Prizes to be given away during the Sweepstakes Period.
4. HOW TO PARTICIPATE: Sweepstakes are offered electronically via the Internet.
NO PURCHASE OR ACTION OF ANY KIND IS NECESSARY TO ENTER OR WIN. ALTERNATE MEANS OF
ENTRY (“AMOE”): Entrants may receive one (1) entry by legibly printing the Entrant’s name, street address,
city, state, zip code, telephone number, and complete email address, on a piece of paper and mailing it to Fuel
Cycle, Inc. Attn: The Cleveland Clinic Patient Perspectives Sweepstakes, / [Insert Sweepstakes Type] (e.g.
Activity Sweepstakes, , Recruiting Sweepstakes), Month or Time
Period of Sweepstakes (e.g. September 1st 2022 – October 1st 2022 ) & Study Name (applicable for Activity
Sweepstakes only), 11859 Wilshire Blvd., Suite 400 Los Angeles, CA 90025. All entries become the exclusive
property of Fuel Cycle and will not be acknowledged or returned.
Eligibility for each Sweepstakes is defined as follows:
a) Activity Sweepstakes – Entrants may enter the Contest by following the link in the Entrant’s
invitation to the Cleveland Clinic Patient Perspectives website, and completing the activity online as
specified in the email communication. These Entrants will receive one entry during the Contest
Period, unless otherwise specified in the email communications.
c) Recruiting Sweepstakes – Entrants may enter the contest if they 1) complete the screening
questionnaire, 2) qualify to join the Cleveland Clinic Patient Perspectives Community, and 3) activate
their membership in the community by clicking the activation link they receive via email after they
complete the screening questionnaire. These Entrants will receive one entry during the Contest
Period.
5. SWEEPSTAKES DRAW(S): Draws will be held at: 11859 Wilshire Blvd., Suite 400 Los Angeles,
CA 90025 on or around on the first business date after the sweepstakes period ends (“Draw Date”). Winners will be determined by random drawing from all eligible entries received by Fuel
Cycle. The odds of winning are the same for all Entrants regardless of whether an Entrant entered the
Sweepstakes online or through the AMOE.
6. PRIZE AWARDING: Prizes will be awarded only on verification by Fuel Cycle that all Contest
requirements have been met by the winners. Each Prize must be accepted as awarded and no substitution,
transfer, conversion or assignment of Prizes will be allowed, unless at the discretion of Fuel Cycle which may
substitute a Prize of comparable or greater value. No correspondence will be entered into except with selected
Entrants at the email address listed on the Fuel Cycle systems, or on the mail-in entries. Fuel Cycle will notify
each winner by email within about fifteen (15) days of the relevant Draw Date. Winners agree to Fuel Cycle’s
Terms & Conditions; (iii) confirms compliance with these Contest Rules and Regulations; (iv) releases and
forever discharges Fuel Cycle, its affiliates and subsidiaries and its advertising and promotional agencies from
all claims, demands, damages, actions and causes of action arising or to arise by reason of the acceptance
or use of the Prize; and (v) gives permission to Fuel Cycle, at its option, to publish or otherwise use the
Entrant’s name, address, photograph, voice and comments, without compensation, in any publicity carried out
by Fuel Cycle or its advertising agencies.
7. SWEEPSTAKES GENERAL RULES: By entering the Sweepstakes, Entrants agree to abide by
these Sweepstakes Rules and Regulations and the decisions of Fuel Cycle, which are final. VOID WHERE
PROHIBITED BY LAW. U.S. only promotion. The laws of Ohio shall govern this Sweepstakes. Entrants
consent to the state and/or federal courts venued in Cuyahoga County, Ohio for any action arising out of this
Agreement. Fuel Cycle management is the final judge on any decisions that are made regarding the
promotion. Winners need not be present when drawing for prizes. Fuel Cycle reserves the right to change,
cancel, shorten or extend the promotion at any time without notice.
8. WINNERS LIST: To obtain a copy of the winner list or a copy of the Sweepstakes Rules and

a self-addressed stamped envelope.
9. DISQUALIFICATION: Fuel Cycle is not responsible for lost, interrupted or unavailable network
server or other connections, miscommunications, failed telephone or computer or telephone transmissions or
technical failure, jumbled, scrambled or misdirected transmissions, or other error of any kind whether human,
mechanical or electronic, including AMOE entries through the mail. Persons found tampering with or abusing
any aspect of this Sweepstakes or the operation of Cleveland Clinic Patient Perspectives website or any other
Fuel Cycle hosted website, as determined by Fuel Cycle, in its sole discretion, will be disqualified. If
disqualified for any of the above abuses, Fuel Cycle reserves the right to terminate the Entrant’s eligibility to
participate. In the event any portion of this Sweepstakes is compromised by virus, bugs, non-authorized
human intervention or other causes beyond the control of Fuel Cycle, which in the sole opinion of Fuel Cycle,
corrupts or impairs the administration, security, fairness or proper entry into the Sweepstakes, Fuel Cycle
reserves the right, in its sole discretion, to suspend or terminate the Sweepstakes. In no event will more Prizes
